Invitation to partner: Building a holistic view of social care need

We want to develop a holistic view of social care need around an individual. This responds to the current challenge of there being no simple way for frontline professionals to see all the services that a resident engages with. We’re keen to explore how to enable cross-team working and reduce time spent on admin tasks by social workers. 

We successfully tackled a similar challenge in children’s social care with our Family Context project. We’d love to see if we could use these learnings to help address similar themes in adult social care.
